Transition Program
This program is designed for teenagers with congenital heart defects and their parents. It is directed for those patients who will be transitioning from care at pediatric centers to hospitals with an adult congenital heart program. The transition program provides information regarding the transition from one hospital to another as well as provides information about adult clinic. They will also be given information about CCHA how to join and more information about our aims and mission.
